How Do I.... Register With The Practice?

If you would like to register with the practice, please bring your medical card with you (if you have one) or fill in a form available at reception. We will confirm you live in the practice area and then ask you to complete a form detailing your medical history. The receptionist will be able to help you with any questions you may have.

If you are on regular medication you will need to make an appointment with our practice nurse and a doctor who will undertake a registration health check. This appointment will involve a discussion of your general health and past medical history, the measurement of your weight and blood pressure and a urine test. This is important as it may take some time for records to be transferred from your previous doctor.

Medical treatment is available from the date of registration.
