New Patient Registration

In order to process your registration, we need you to bring in the following documents:

  • Purple NHS Registration Form GMS1. (Download form here)

  • An Adult Registration Form, a Child Registration Form or an Under Five Child Registration Form.

  • Photo identification such as a valid passport or driving licence.

  • Proof of your address: This can be a letter such as a formal lease document, tax statement or utility bill, these must be dated within the last three months. A bank statement will only be accepted as a last resort and will need to show a pattern of spending in the UK.

  • If you have arrived from another country, we need to see your passport and valid visa or work permit if applicable.

  • All patients aged 11 and over require a new patient check.

  • You will need to complete a registration form and bring with you two forms of identification before you are able to start using Emis Patient Access to order repeat prescriptions online, make appointments online and access your medical records online.

To complete your registration you will need to have a new patient check, at which time you will need to provide a fresh urine sample. You can collect a specimen bottle from Reception and hand in a fresh urine sample any day before 12 noon (or bring it with you at your new patient check appointment).
